Non-communicable diseases, including hypertension and diabetes mellitus, are the leading cause of global death and pose the greatest health burden in Indonesia. This situation demands strengthening prevention and early detection at the community level. This community service activity aims to improve health knowledge and conduct early detection through health education and examinations in RW 06, Kalibaru Village, North Jakarta. The method used was a Community-Based Research (CBR) approach, actively involving the community from planning to evaluation. A total of 52 participants participated in the activity, with 46 respondents undergoing health examinations and 34 respondents completing a knowledge questionnaire. The examination results showed that 80.5% of respondents had abnormal blood sugar levels and 91.4% had abnormal blood pressure. Questionnaire analysis showed that the majority of respondents had good knowledge (58.8%), but there were still groups with sufficient knowledge (29.4%) and insufficient (11.8%). Education, age, and gender factors influence the level of knowledge. The gap between good knowledge and still abnormal health conditions indicates that health education needs to be followed by changes in healthy behavior and lifestyle. This activity emphasized the importance of the role of pharmacists as educational agents and community partners in the promotion and prevention of non-communicable diseases.
